FDA approves targeted therapy for metastatic pancreatic cancer 

Aug. 26, 2026—The Food and Drug Administration today approved daraxonrasib (Rasonque, Revolution Medicines) for the treatment of adults with metastatic pancreatic adenocarcinoma who have received at least one prior systemic therapy or are not candidates for multiagent systemic therapy.

Clues, questions in HR deficiency testing

August 2026—Testing for homologous recombination deficiency (HRD) is not for the faint of heart. Even the experts agree. Tricia Numan, MD, says that when she first became involved in the topic, “I didn’t realize how many components went into HRD testing,” including germline alterations, somatic alterations, and various classifications of genomic instability.

Eyes on new tick-borne disease diagnostics

August 2026—In a summer of more ticks, more diseases, and more tick species expanding to new areas and bringing diseases with them, what Bobbi Pritt, MD, MSc, would like to see are more—and better—diagnostics in early Lyme disease. Laboratory scientists are inching toward improved solutions, though nothing is commercially available now. “I’m cautiously optimistic. I wouldn’t say I’m excited yet,” says Dr. Pritt, co-director of vector-borne diseases laboratory services and chair of the Division of Clinical Microbiology at Mayo Clinic in Rochester.

Urothelial carcinoma: histologic subtypes and divergent differentiation

August 2026—Recognizing and reporting histologic subtypes and divergent differentiation in urothelial carcinoma is critical for diagnosis, prognosis, and treatment. “Ideally, you want to mention the subtype histologies in pathology reports and quantify the amount,” said Francesca Khani, MD, associate professor in the Department of Pathology and Laboratory Medicine and in the Department of Urology, Weill Cornell Medicine. In a CAP25 session last fall, Dr. Khani spoke of the mimics of benign entities and of other malignancies.

Views on AI use for lab test orders and reports

August 2026—Would artificial intelligence as an aid in ordering laboratory tests and making reports understandable to patients be a worry or a welcome step? In a June 2 online roundtable led by CAP TODAY publisher Bob McGonnagle, a Compass Group member shared what he learned about an Epic pilot, and that launched the following conversation about the clinical laboratory and pathology reports that could be made easier for patients to understand. One participant’s view: “proceed with caution.”

Student-led workshops introduce pathology to peers

August 2026—A series of hands-on or role-playing workshops created by pathology student interest group leaders at a California medical school introduced pathology to dozens of students who would have otherwise completed their curriculum without that exposure.

AMP case report: Therapy-related acute myeloid leukemia in a patient with BRCA1-positive breast cancer and a DDX41 possibly germline variant

August 2026—The 2022 World Health Organization and International Consensus Classification frameworks recognize several well-defined myeloid neoplasm categories that arise in the setting of inherited genetic predisposition. These entities are grouped according to associated clinical features and include myeloid neoplasms with germline predisposition in the absence of antecedent platelet abnormalities or organ dysfunction (e.g. CEBPA, DDX41, and TP53), those associated with inherited platelet disorders (e.g. RUNX1, ANKRD26, and ETV6), and disorders linked to underlying or potential organ dysfunction.

Cytopathology in focus—Predictive markers in cytopathology: programmed death ligand-1

August 2026—Although cytologists and cytopathologists expend considerable effort in screening, risk stratification, diagnosis, staging, and prognostication, clinicians are increasingly demanding ancillary testing that predicts response or resistance to the broad range of therapeutic agents available to them. In advanced-stage non-small cell lung carcinoma, these therapies include tyrosine kinase and other small-molecule inhibitors, bispecific antibodies, and antibody-drug conjugates.

Cytopathology in focus—What sets pediatric cytology apart?

August 2026—Pediatric pathology remains a relatively small subspecialty within anatomic pathology. The American Board of Pathology since 1990 has issued only 648 subspecialty certificates in pediatric pathology, and between 2020 and 2024, only 14 to 21 candidates sat for the pediatric pathology certifying examination each year.
