Former CEO’s view from on top—and below

July 2026—Open a new high school. Bypass the insurers. Take a broad definition of health. Bring core services in-house. That is some of what Northwell Health CEO emeritus Michael Dowling did while spearheading Northwell Health’s expansion during his 23 years as president and CEO of what is today New York’s largest not-for-profit health care provider and private employer. Dowling stepped down in October 2025; Northwell executive vice president John D’Angelo, MD, succeeded him.

Grading, staging papillary urothelial carcinoma

July 2026—Grading of papillary urothelial carcinoma can sometimes be challenging because it requires categorizing discretely a continuous spectrum of tumors. In the view of Lara Harik, MD, discovering biomarkers that can predict progression would be an ideal solution for papillary urothelial carcinoma, especially for tumors that are difficult to grade. “Maybe the use of cell-free DNA in the near future,” she said, “could reveal which of these tumors are at risk for progression.”

July 2026—Being able to distinguish the subset of patients with stage IV colorectal cancer who have a chance at cure with ablation or resection from those who do not has been the aim of two University of Chicago physicians who developed a biomarker for colorectal liver metastases called PersonaCRC.

Drawing learners and leaders to pathology: The impact of CAP Foundation awards and grants

July 2026—A medical student walks into the CAP annual meeting for the first time, nervous and not sure what to expect. She’s there because of a Medical Student Travel Award she almost didn’t apply for. By the end of the week, she has an interest in pursuing pathology. That is what the CAP Foundation had in mind when it began in 2018 to offer that student travel award. Now, for the first time, after an analysis of CAP Foundation data and a long-term survey of past recipients of that award and others, we have the data to report their impact.

July 2026—Sepsis is a major cause of pediatric morbidity and mortality worldwide. It is difficult to measure the number of pediatric sepsis cases since most estimates are based on administrative codes, and coding practices vary by institution. Medical record review offers greater accuracy in recording pediatric sepsis diagnoses, but it is labor intensive and subject to interrater variability.

July 2026—Hepatoid lung carcinomas, similar to hepatoid carcinomas of other sites, are defined as extrahepatic tumors exhibiting divergent hepatocellular differentiation. Uniquely, hepatoid carcinomas of the lung are reported to commonly express only HepPar1, which recognizes the mitochondrial enzyme carbamoyl-phosphate synthetase-1 (CPS1). HepPar1/CPS1 was recently found to accumulate in lung adenocarcinomas (LUAD) harboring STK11 mutations, presumably as a genotype-associated metabolic adaptation.

July 2026—Parkinson’s disease, which is characterized by loss of dopamine-producing neurons in the brain’s substantia nigra and the pathologic misfolding and accumulation of alpha-synuclein, can result in a range of motor and nonmotor symptoms that may vary by individual. The cardinal motor symptoms of Parkinson’s disease (PD) are bradykinesia (slowness of movement), resting tremor, rigidity, and postural instability. A prodromal phase of the disease can generate nonmotor symptoms, such as a reduced sense of smell years prior to the onset of cardinal motor symptoms.
