Clinical pathology selected abstracts

August 2026—Patients undergoing cardiac surgery have a higher risk of excessive bleeding and complications due to dilutional effects during surgery and activation and consumption of platelets and coagulation factors. Increasing recognition of the related morbidity and mortality has heightened interest in patient blood management, including autologous blood salvage (ABS) using an automated cell-salvage system for safe and effective blood conservation in cardiac surgery with cardiopulmonary bypass. Of importance in adult and pediatric populations is that reinfused washed red blood cells be of high quality to prevent adverse events from cytolysis and activation processes and severe and fatal complications.

August 2026—Primary perianal adenocarcinoma of intestinal type has been described in recent literature and proposed as a subtype of extramucosal anal adenocarcinoma. Whether this represents a unique entity remains to be elucidated. The authors conducted a study in which they analyzed the clinicopathologic and genomic features of 14 cases of primary perianal adenocarcinoma of intestinal type (PPAI).

August 2026—Kawasaki disease is an acute vasculitis of childhood and the most common cause of acquired heart disease in children worldwide. Up to 25 percent of untreated patients develop coronary artery aneurysms. The variable clinical presentation of Kawasaki disease (KD) complicates diagnosis, treatment, and understanding of the disease and highlights the need for molecular tools to stratify patients into biologically meaningful subgroups.

Q. Are laboratories sticking to the established standard of spinning whole blood coagulation test tubes within four hours of specimen collection or are most changing to a 24-hour window (with some exceptions) per Clinical and Laboratory Standards Institute guidelines? Read answer.
Q. I am evaluating a large, well-circumscribed necrotic pelvic peritoneal implant in a case of serous borderline tumor (SBT) of the ovary. The implant shows nonviable necrotic tissue displaying only residual ghost papillary outlines with calcification. My initial interpretation is the peritoneal nodule is an invasive implant because it is large. I am assuming the necrosis represents infiltrative, destructive, and expansile neoplastic cells that form the large nodule (17 mm). I considered upgrading the SBT to low-grade serous carcinoma (LGSC) because of the presence of this implant. However, since the implant is necrotic, I cannot reliably evaluate for tumor cells. In addition, it is rare to have an invasive implant without an invasive component at the primary site. What is your opinion regarding this implant? Read answer.
